Total Motile Count (TMC) is the total number of moving sperm in an ejaculate sample. It's calculated by multiplying volume × concentration × motility percentage. TMC is one of the most important indicators of male fertility because it represents the actual number of functional sperm available for fertilization. A higher TMC generally indicates better fertility potential.

TMC is a critical predictor of natural conception success and IUI outcomes. Unlike individual parameters like count or motility alone, TMC provides a comprehensive measure of functional sperm. Studies show that TMC correlates strongly with pregnancy rates. For natural conception, a TMC above 40 million is generally considered optimal, while values below 20 million may indicate reduced fertility potential.

TMC reference ranges are: Excellent (>80 million), Normal (40-80 million), Low-Normal (20-40 million), Borderline (10-20 million), Low (5-10 million), and Severely Low (<5 million). However, pregnancy can still occur with lower values, and many factors beyond TMC affect fertility. These ranges are guidelines, and individual circumstances vary significantly.

Improving TMC involves addressing the underlying parameters: volume, concentration, and motility. Lifestyle changes like maintaining a healthy weight, exercising regularly, avoiding smoking and excessive alcohol, reducing stress, and eating a nutrient-rich diet can help. Supplements like antioxidants, zinc, and folate may improve sperm quality. Avoiding heat exposure to the testicles and treating underlying medical conditions are also important. A man might have a high concentration but low volume or poor motility, resulting in a low TMC despite seemingly normal individual values.
